About the Role
TheBarnet Children’s Continuing Care Team is a small, supportive, and highly committed team of very experienced nurses
responsible for coordinating and overseeing NHS Funded Continuing Care packages for children and young people with
complex and long-term health needs.
Working in line with theNational Framework for Children and Young People’s Continuing Care (2016), the team ensures that
children and young people receive safe, high-quality, and personalised care that enables them to live as well as
possible within their homes and communities.
