About the Role

To provide clinical guidanceto the team members of the Continuing Healthcare teams and others. ii)To ensure that nursing

and continuing healthcare assessments, undertaken by the team members, are accurate. iii)To carry out the relevant NHS

Continuing Healthcare assessments and reviews, care planning for allocated clients on the caseload. iv)To participate

and represent Tower Hamlets and Newham on the Continuing Care Appeals panel. v)To adhere to the NMC Code of Professional

Conduct and Fitness to Practice, ELFT and LBTH adult policies and procedures at all times and ensure that all managed

staff are compliant. vi)To provide effective case management for complex neurological patients in specialists placements
