Crisis Intervention Nurse
About the Role
Work as an autonomous practitioner providing specialist mental health assessments for children and adolescents
presenting in crisis, including those with complex co-morbid conditions and high-risk behaviours. Conduct thorough risk
assessments (including self-harm and suicide risk), formulate crisis and contingency plans, and implement positive risk
management strategies. Deliver timely follow-up appointments and ensure safe discharge planning with documented care
plans recorded on the electronic patient record. Facilitate referrals to appropriate internal and external services and
ensure continuity of care. Coordinate care for CYP admitted to Paediatric Wards, liaising with Paediatric Liaison Nurses
