About the Role

The post holder will provide specialist dementia expertise to the multidisciplinary team to ensure that patient care is
safe and of high quality. To work collaboratively across professional and organisational boundaries to provide a
holistic and comprehensive assessment of patient need and ensure appropriate care and timely and safe discharge. To
provide education and training to members of the multidisciplinary team to raise awareness of dementia, delirium and
