About the Role

JOB PURPOSE This role provides specialist dementia expertise within Ashfield North PCN. It is open to Registered Nurses

with dementia specialism The post holder will: Deliver specialist, relationship centered dementia care. Support people

with dementia and their families across the full dementia pathway from early diagnosis to end of life care. Provide

expert assessment, complex case management, and evidence based interventions. Offer clinical leadership, education, and

support to PCN staff and system partners. Contribute to service development, pathways, and dementia strategy work. To

see patients and or families in GP Surgeries, patients homes, or other locations. KEY RESPONSIBILITIES 1.Clinical Duties
