About the Role
Main Duties and Responsibilities: Frailty prevention - Support frail people to remain independent for longer by
providing evidence-based Comprehensive Geriatrics Assessments (CGAs) in the community responding cohesively alongside
the wider MDT. Integrated neighbourhood working - Regular attendance at the Integrated Neighbourhood team MDT. When
required supporting the smooth running of these meetings as Chair. Clinical Leadership & Frailty Care Work cohesively
with the wider MDT, including Palliative and EOL care teams in the community, to coordinate holistic, person-centred
care and ensure[EF1] smooth transitions across pathways. Assist in monitoring and auditing clinical practice to ensure
