Deputy Team Leader

About the Role

The post holder will be responsible for providing support to the Team Leader in the day to day management of

multi-disciplinary/multi-agency packages of scheduled and unscheduled care for patients with multiple, complex and

palliative care needs. Assisting the Team Leader in the requirements of the nursing team by utilising effective

management and leadership skills. Responsible for the delegation of clinical nursing activities to appropriately skilled

team members, ensuring that an efficient and effective service is delivered to the practice population. Responsible for

prioritising delegated home visits according to the patients clinical need and geographical location.
