Diabetes Clinical Pharmacist
About the Role
Provide expert pharmaceutical leadership within the Community Diabetes Service, ensuring safe, effective, and
evidence-based use of medicines while promoting high standards of clinical practice and patient care. Lead and
collaborate in multidisciplinary care, working closely with diabetes specialist nurses, dietitians, consultants, GPs,
and other health professionals to deliver integrated, patient-centred community care. Drive medicines optimisation and
clinical quality, supporting improvements in patient outcomes, safety, adherence, and alignment with national and local
best practice guidelines and pathways. Undertake structured medication reviews, including complex patients with
