About the Role

A fantastic opportunity has arisen for a Diabetes Specialist Nurse to join our East Cambs & Fenland Team working full

time 37.5 hours per week. If you have compassion, empathy with the ability to work autonomously, this is the opportunity

for you.

You will work as part of the multidisciplinary team with the aim of preventing admission and providing ongoing care for

patients with Diabetes Disease within the community.

You will also be responsible for managing a caseload of patients with complex Diabetes needs, using evidence based
