About the Role

We have an exciting opportunity for a Band 7Diabetes Specialist Nurse to join our award-winning proactive Hounslow

Community Diabetes Service.

We are seeking an experienced Diabetes Specialist Nurse who has the passion and skills for improving the care of people

with Type 2 Diabetes (and Type 1 Diabetes in special circumstances). The role would suit someone who is highly motivated

with a positive approach to high quality care delivery with excellent communication and leadership skills to work in

partnership with our community and primary care colleagues.
