About the Role

Imperial College Healthcare NHS Trust seeks a motivated Band 7 Diabetes Specialist Practitioner to join our

multidisciplinary team across five hospitals in Northwest London. Based mainly at Hammersmith Hospital, this full-time,

on-site role involves delivering advanced clinical care, managing complex diabetes cases, leading service improvements,

supporting staff, and providing education on medication and pathway development.

The post-holder will be required to cover any of the other sites in the event of an emergency though will be based at

Hammersmith Hospital site. The post is required to provide a 5-day service from Monday to Friday on site.
