About the Role

Duties and Responsibilities This role is primarily focused on the management of patients with diabetes within the

practice. The postholder will be responsible for delivering high-quality diabetes care and working closely with GPs and

the wider nursing team. There will be the opportunity to carry out other practice nursing duties depending on experience

and interest. Support and training will be provided where needed, but the main emphasis of the role will remain diabetes

management.Main Responsibilities Diabetes Provide routine reviews and ongoing management for patients with Type 2

diabetes and pre-diabetes Work in line with current guidelines and QOF requirements Support patients with lifestyle
