About the Role

We are seeking a proactive and highly motivated individual to join the Discharge Team at Frimley Park Hospital as a

Discharge Care Coordinator.

The post-holder will play a key role in supporting safe, timely, and well-coordinated patient discharges, working both

autonomously and as an effective member of the multidisciplinary team.

The successful candidate will be enthusiastic, well organised, and able to manage a varied workload in a fast-paced

environment.
