Discharge Co-ordinator
About the Role
In this role, you will be an integral part of our positive, committed team across the Birmingham division. You will play
a key role in supporting safe, timely and effective patient progression across the Birmingham site. Working closely with
the Discharge Liaison Nurse, the postholder will coordinate discharge activity, strengthen patient flow, and contribute
to reducing length of stay through proactive planning, communication and partnership working. You will support
multidisciplinary teams MDTs, external agencies and internal services to ensure that every patient has a clear, well
managed pathway and that delays are identified, escalated and resolved As part of our forward thinking service, you will
