About the Role
The Discharge Coordinator will lead and coordinate complex discharge planning within a multi-disciplinary team, ensuring safe and timely discharges for patients.
Responsibilities
- Lead discharge planning for complex cases.
- Conduct person-centered assessments.
- Collaborate with multi-disciplinary teams.
- Manage patient health and social care needs.
- Participate in service development initiatives.
Requirements
- Registered on Sub-part 1 or 2 of the UK NMC Register.
- Experience in assessing older patients.
- Strong clinical judgment and decision-making skills.
- Excellent communication and interpersonal skills.
- Ability to adapt and manage change effectively.

Location & Schedule
