Discharge Co-ordinator

About the Role

The Discharge Coordinator will lead and coordinate complex discharge planning within a multi-disciplinary team, ensuring safe and timely discharges for patients.

Responsibilities

• Lead discharge planning for complex cases.
• Conduct person-centered assessments.
• Collaborate with multi-disciplinary teams.
• Manage patient health and social care needs.
• Participate in service development initiatives.

Requirements

• Registered on Sub-part 1 or 2 of the UK NMC Register.
• Experience in assessing older patients.
• Strong clinical judgment and decision-making skills.
• Excellent communication and interpersonal skills.
• Ability to adapt and manage change effectively.

Compensation

The salary for this position ranges from £41,608 to £50,702 per annum, pro rata.

Location & Schedule

This role is based at Perth Royal Infirmary, with working hours of 30 per week.
