About the Role
Discharge Facilitator
Band 6,
The post holder will be responsible for case managing and facilitating safe and effective discharges of patients from
hospital to a community setting through a multidisciplinary approach. They will coordinate oversee Discharge Navigators
within their sub-team, complete assessments of patients needs and support the discharge plan. The post holder will be
proactive in identifying concerns or issues that may delay effective discharges and work with the Transfer Of Care Hub
