About the Role

We have an exciting opportunity for a Discharge Support Worker with administrative experience to join our team on a busy

35-bedded short stay ward.

This ward experiences a high turnover of patients, providing a dynamic and fast-paced working environment. You will have

the opportunity to work closely with the ward’s Integrated Discharge Team (IDT) Facilitator to support safe and timely

discharges.

The ward is short stay unit with high turnover of patients, opportunity to work alongside IDT facilitator for the ward.
