About the Role
The Discharge Facilitator will support discharge planning for older patients, ensuring efficient patient flow and timely discharges. This role requires collaboration with medical, nursing, and therapy teams to assess discharge needs early in the admissions process.
Responsibilities
- Develop individualized discharge plans for patients.
- Collaborate with multidisciplinary teams for discharge assessments.
- Liaise with community services and families for continuity of care.
- Monitor and reduce delayed discharges across the Unit.
- Communicate effectively with hospital and external agencies.
Requirements
- Experience in a healthcare setting preferred.
- Strong IT skills and ability to prioritize workload.
