District Nurse / Case Manager | Lincolnshire Community Health Services NHS Trust

About the Role

Lincolnshire Community Health Services are seeking a full time District Nurse / Case Manager to join them within the

Lincoln North Community Nursing Team. The team are based at Ravendale Health Centre in the Lincoln North area.

Applicants should hold the Community Specialist Practitioner qualification and have experience of managing a caseload of

patients and leading a team of staff in delivering excellent patient care. Alternatively a Case Manager without the

DNSPQ may be considered for the right candidate with a minimum of 2 years experience within a Community Nursing Team and

the willingness to undertake the DNSPQ course.
