About the Role
JOB DESCRIPTION
The jobholder will assist the Integrated Care Team Manager with the implementation and delivery of an integrated model
of care for one of seven geographical teams. They will work very closely with social work, physiotherapy and
occupational therapy lead colleagues to ensure there is a co-ordinated and integrated approach to the assessment,
delivery and monitoring of high quality and safe care. As the Professional Lead District Nurse the jobholder will be
responsible for the delivery of high quality district nursing services and the day to day supervision of the team’s
