About the Role

We are looking for a Band 6 District Nursing Caseload Holder who would relish the challenge of working within the

District Nursing Service and caring for patients with complex needs whilst working towards an integrated healthcare

structure. We are looking for candidate’s who are self-motivated, keen to work in a progressive division and to develop

their skills and knowledge in supporting patients who are housebound with long term conditions and complex needs.

The service operates 24 hours per day, 365 days per year to cover our 8am-8pm 7 days a week model.

The post holder will have continuing responsibility for the management of a defined caseload. They will be responsible
