Full time - 37.5 hours per week
The District Nursing Practice Facilitator will support the continuous improvement within district nursing teams,
focusing on caseload management, strengthening clinical practice, operational effectiveness, workforce capability and
patient experience. The post holder will work alongside District Nursing Team Leads and Matrons to identify challenges,
