District Nursing Sister/Senior Staff nurse
About the Role
Provide and maintain a high standard of skilled, holistic nursing care for patients in their homes, or a suitable
alternative setting, using an agreed model of nursing care and employing appropriate additional professional skills e.g.
active nurse prescribing. Assess, develop, implement and evaluate programmes of care to meet the holistic needs of
individuals, families and priority groups identified within the practice population. Participate in recognised health
promotion/surveillance programmes, and be pro-active in instigating health improvement measures appropriate to NSFs and
the GMS contracts. Actively raise patients health issues through communication with the Primary Health Care Team and
