About the Role
The Dual Diagnosis and Outreach Team ( DDOT) was created with Community Mental Health Transformation funding in 2021 to
improve access to mental health services for clients with serious mental illness and multiple, complex needs who
struggle to gain access to mental health services via more formal routes.
The DDOT is a small team with a Team Lead, a senior nurse and 2 support workers. The team are an outreach arm of the
Primary Care Mental Health Service (PCMHS) which works closely with GPs to ensure that people can access appropriate
mental health care.
