About the Role
JOB OVERVIEW
The Discharge Team Manager leads and manages the Discharge Coordinator Team and provides operational leadership for the
Denmark Hill Internal Flow Hub, overseeing a multidisciplinary workforce of nurses, social workers and therapists who
support safe and timely discharge processes. The role ensures effective, person‑centred and well‑coordinated discharge
planning across all Divisions, working closely with Trust colleagues, community health services, primary care, local
authorities and wider partner organisations to improve patient flow and the overall discharge experience. A key
