Enablement Support Worker
About the Role
You will undertake a key role in the development/amendment of support plans to deliver assessed needs of service users
within the Short Term Assessment Review Team. Be proactive in support planning, monitoring service users target outcomes
to ensure they are appropriate and up to date to achieve goals, risk assessments, management plans and case
notes/communication sheets. Deliver support which enables carers to continue caring and empower the enablement of
service users. Support personal care tasks including washing, bathing, maintaining personal hygiene, continence care,
nutrition and fluid intake. Promote mobility and independence to support delivery of enablement targets. Pushing and
