About the Role

An exciting opportunity has arisen for a compassionate, motivated, and experienced individual to join our dedicated SWAN

Bereavement Team within the Northern Care Alliance.

Working in alignment with theSwan model of care, you will provide discreet, empathetic support to individuals and

families during the immediate stages of grief—regardless of age, cause, or place of death. This role involves education,

support and empowering healthcare professionals in delivering practical and emotional care, including memory-making,

guidance, and information before and after the death of a loved one
