Job overview An exciting opportunity has arisen for a Palliative care End of life Facilitator to complement our existing
COMPASS team 9Community Palliative advise and support service) The post based within Waltham Forest is to support our
existing team in raising awareness of local and national EOL initiatives Working closely with Care homes across the
borough the successful applicant will develop and maintain links with commissioners and key stakeholders throughout
Waltham Forest Working across the borough the successful applicant will support in delivering Education and will act as
a point of contact for the Community Nursing Team Applicants are welcome for Registered Practitioners with experience in
Responsibilities
