Enhanced Care Home Support Service Visiting Nurse

About the Role

Job Purpose: The post holder will be a named nurse for their identified care homes. The post holder will delegate the

workload to the Enhanced Care Home Support Nurses and healthcare assistants as appropriate and as defined by the

priority/acuity of the caseload and liaise with all health care professionals and other agencies within the wider

community to ensure effective service delivery. The post holder will work under the direction of the Clinical Lead for

the service and will be responsible for the assessment of patient need. The post holder will work to develop the service

and extended MDT. The post holder will develop, advise, and evaluate programmes of care within the care homes. The post
