About the Role
JOB DESCRIPTION
The Complex/Enhanced Care Service primary focus is on maintaining people within their own home or place of residence who
require complex nursing and health care interventions including the management of long term conditions, an unexpected
deterioration in health or in the event of a crisis. The service aims to prevent in-patient admissions to hospital,
reduce length of hospital stay and facilitate early discharge. The post holder will be required to work collaboratively
and in partnership with the multi-disciplinary team and to promote and monitor safe and effective nursing practice and
