About the Role

Work collaboratively as part of the PCN multidisciplinary team Support care homes and the frail elderly population

across the PCN Undertake comprehensive assessments of patient need Develop, implement and evaluate complex care plans in

patients homes and/or care homes Consult with patients, families and the wider multi-professional team Provide

supervision and support to trainee nurse associates, nurse associates and general practice nurses Maintain accurate,

high-quality patient records and manage referral/care pathway databases Participate in clinical audits and research

projects Develop, update and implement local practice protocols, guidelines and procedures Build effective working
