Enhanced Practice Mental Health Nurse
About the Role
Clinical Practice Deliver timely, personcentred care for adults with low to moderate mental health needs referred from
within the PCN, whether through GP triage, direct referral, or proactive outreach. Conduct comprehensive mental health
assessments and develop shared care plans with patients. Deliver brief interventions, psychoeducation, and proactive
medication support (where appropriately trained). Refer and signpost to IAPT, CMHTs, crisis services, and VCSE
organisations as required. Work collaboratively with GPs, ANPs, Social Prescribers, OTs, and other team members to plan
joinedup care. Work autonomously and confidently to manage complexity and psychosocial need. Apply holistic clinical
