About the Role
Job Purpose The Practice Nurse will provide and maintain a high standard of nursing care for patients, offering advice,
health education, and support to promote better health and wellbeing. Working as part of the multidisciplinary team, the
post-holder will manage a range of nursing duties, including chronic disease management, vaccinations, cervical
screening, wound care, and health promotion, while contributing to the continuous improvement of clinical services
