Family and Community Support Worker

About the Role

The role entails but is not limited to: Clinical Practice The post holder will work under supervision and in

collaboration with other members of the team to: Work alongside staff and volunteers to provide culturally sensitive

psychological, emotional, social and spiritual support for families and patients within the In-patient Unit and within

the Community Nursing Team Co-ordinate and be involved in the provision of an education programme In collaboration with

the Family and Community Support Service Lead, oversee a team of volunteers to coordinate and facilitate a community

wellbeing programme Maintain up-to-date confidential and accurate records using electronic systems and fulfil any
