About the Role

Children’s Continuing Care Team - Family Support Worker Band 4.

The Children’s Continuing Care Team provides specialist care to children and young people with complex health needs in

the community setting 24/7 across Derby and Derbyshire. Therefore the ability to drive/travel independently is

essential.

We are looking for motivated, enthusiastic Band 4 Family Support Workers to join our team. The Band 4 position is a

senior role and requires experience working with those receiving long term ventilation and complex health needs. These
