Fast Track Senior Nurse Assessor

About the Role

Working within the funded care team in the ICB, this fast paced and varied role, provides the opportunity to use

clinical judgement, analysis of information, and heightened communication skills to support rapid access to NHS funded

care. As part of a team of nurses, the successful candidate will process fast track applications, liaising with patients

, their representatives, and both community and acute trust sectors to ensure care needs are met and the patient is

supported through their end of life journey. The successful candidate will possess excellent communication and

interpersonal skills, a good working knowledge of end of life care, the ability to problem solve and make robust
