About me I live in the community in my own home, with no access to the outside due to my health conditions. My main

diagnosis is Complex Regional Pain Syndrome (diagnosed in 2008) which means I suffer from excruciating pain daily. This

means I am largely bedbound and I am unable to carry out daily tasks such as cooking, cleaning, shopping, organising my

home etc. With the right support in place, I wish to improve my health as much as possible, to the point of being able
