About the Role

Clinical care, including: interventions, medications, assessment, wound care and management, cervical screening, ear

care, vaccines, pre-travel health, prescribed therapies Health promotion, including: alcohol screening, smoking

cessation, healthy diet, physical activity, weight management, sexual health and family planning Sexual health,

including: health promotion, investigation and contraception Long term conditions, including: screening, diagnosis,

ongoing management, and referral to other agencies in line with NICE guidance and Quality and Outcome Framework

indicators Develop and follow referral pathways Risk stratification and care planning Quality assurance and service
