About the Role

The Frailty Follow Up Nurse plays a vital role in providing clinical leadership and support within the ward. This position requires a registered nurse with a minimum of two years' experience in acute or community settings, ensuring high standards of evidence-based nursing care while promoting patient involvement and staff development.

Responsibilities

Requirements
