Liaise and collaborate with medical, nursing, therapy, and social carecolleagues across the urgent care pathway to

ensure coordinated, high quality care. Represent the Healthy Ageing Service and individual patients

withinmultidisciplinary and multiagency meetings, contributing specialistoccupational therapy advice on assessment,
