Primary Care Network in Woodberry Wetlands. This post will be based across 4 GP practices and will be hosted by the

Adult Community Rehabilitation Team at Homerton Healthcare.

This forward thinking role is ideal for an experienced OT passionate about proactive care, prevention, supporting

clients with rising needs and community-based frailty rehabilitation.
