About the Role
To work under the direct supervision and guidance of the nursing team. Acts on own initiative in delivering patient care
in the community. To support multi-disciplinary health professionals in providing a comprehensive rehabilitation
programme to prevent avoidable hospital admission. To provide care and rehabilitation for service users within their own
homes under the clinical direction of qualified staff. This includes assessing patients condition through observation
and clinical skills, recording appropriately on EMIS and ensuring the relevant professionals are informed in a timely
manner. Communicate factual information to patients/carers that requires patience, persuasion and reassurance. To
