General Nursing Assistant (GNA) Care Coordinator
About the Role
The GNA Care coordinator will continue to develop and undertake a range of delegated tasks and will report to a
Registered Nurse or Practitioner. The GNA Care coordinator will be expected to provide general/specific care as required
for our patient group of both rehabilitative and end of life patients and will be proficient and competent to work
across professional disciplines, referring clients for assessment and treatment or to other agencies as required. The
GNA Care coordinator will be an essential member of the Integrated Placement of Care Hub in assisting to provide the
urgent and planned care to patients in their own homes and care homes preventing avoidable hospital admissions and
