About the Role

Duties and Responsibilities are: The post holder will make decisions, provide safe, evidence-based, cost effective,

individualised patient care. Clinical : To undertake all duties inline with the training and instruction provided by the

general practice team. Responsible for the management of patients with LTC(s) by ensuring the smooth running of chronic

disease clinics, including clinical, emotional, physiological reviews and support Responsible for clinical and

administrative pathways to provide patients with continuity, coordinated, informed, planned care Assist patients with

their medications, compliance and check any equipment used, such as inhalers, are working effectively Responsible for
