About the Role
Job Summary The postholder will provide high quality primary care services to the practice population, managing a broad
and varied caseload. They will deliver patient centred clinical care in line with evidence based practice, professional
standards, and contractual requirements.Key ResponsibilitiesClinical Duties Conduct face to face, telephone, and online
consultations. Assess, diagnose, treat, refer, and manage patients with undifferentiated and complex conditions. Provide
chronic disease management in accordance with practice protocols. Undertake home visits when clinically appropriate.
Review and sign prescriptions; manage repeat prescription queries. Interpret and act on test results, correspondence,
