About the Role

We’re seeking a motivated GP with a passion for frailty care to join our dynamic West Kent Home Treatment Service. This

is an exciting opportunity to work at the heart of an expanding multidisciplinary frailty team, delivering high‑quality,

proactive care to patients in their own homes and community settings.

In this role, you’ll work closely with Primary Care, Secondary Care, Social Care, Mental Health services and

Commissioners to provide a responsive, integrated service for people living with frailty. You’ll play a key part in

preventing unnecessary hospital admissions, supporting timely intervention and improving quality of life for some of our
