About the Role
This role helps people at risk of falls by supporting safe, timely care that improves confidence and independence.
You’ll work as part of an experienced integrated team, supporting fracture liaison nurses, therapists and other
colleagues. You’ll visit people in their homes and care settings, helping them get the right support at the right time.
Your work will make a real difference to people’s daily lives, especially those with complex needs.
What you’ll do:
• Register new patients and support the team to identify people who may benefit from the service
