About the Role

Undertakeplanned and same-day PCN home visitsfor registered patients across Enfield South West PCN Provide holistic

clinical assessment, diagnosis, and management in the home setting Work closely with thePCN MDT(paramedics, nurses,

pharmacists, care coordinators) Liaise with practices to support continuity and reduce same-day workload pressure

Complete concise clinical documentation and appropriate follow-up actions Participate in service development, audit, and

quality improvement (light-touch)
