About the Role
JOB OVERVIEW
The post holder will lead and manage the Discharge Coordinator Team and act as the clinical lead within the Transfer of
Care Bureau, facilitating effective multidisciplinary discharge planning across all Divisions.
They will work in partnership with Trust staff, external agencies, community and primary care, and other providers to
improve patient outcomes and discharge experience, while supporting patients, families or carers, and staff to safely
resolve complex cases, including high- and medium-risk discharges.
