HCA - Community palliative care

About the Role

Community palliative care provides advice, support and symptom management to patients with a life limiting diagnosis The

community palliative care team team provides specialist nurse led care in patients own homes. They visit patients after

receiving a referral from a healthcare professional to plan ongoing care and treatment as appropriate. The Band 3 Health

Care Assistant will work as part of the community palliative care team to provide support and advice to patients at

home. KEY RESPONSIBILITIES CLINICAL It is accepted that at times, the HCA works without direct supervision and the RGN

acknowledges accountability of all delegated care. Provision of high quality care to individuals, families, and groups
